Written notice within 15 days of accident unless employer had actual notice (§50-6-201)
Filing with the state
When employer has not paid benefits, right is forever barred unless notice required by §50-6-201 is given AND petition for benefit determination is filed with bureau within 1 year after accident resulting in injury (§50-6-203)
